There are many things to consider when there’s an injury to the mouth:
- Are teeth involved?
- Are there bone fractures?
- Are the affected teeth primary (baby) or permanent?
If Primary (Baby) Teeth Are Involved
- Nerve Exposure
- If the fracture reaches the nerve and the tooth is fully developed, we may be able to treat it with a pulpectomy and place a crown.
- Root Fracture
- Horizontal fractures that don’t impact surrounding tissues may not need treatment.
- Vertical fractures often require extraction.
- Tooth Development
- Treatment depends on how developed the tooth’s roots are.
- Underdeveloped roots are more likely to require extraction.
- Tooth Intrusion (Pushed In)
- If the tooth is pushed in but not fractured, no immediate treatment may be needed. Allow it to re-erupt naturally, with monitoring.
- Tooth Avulsion (Knocked Out)
- Do not reinsert a completely knocked-out baby tooth.
Common Causes of Injury
- We often see injuries from scooters, skateboards, and falls from monkey bars. Encourage the use of helmets and mouthguards to help prevent accidents!
If Permanent Teeth Are Involved
- Fractured Tooth
- If a piece of the tooth is broken but the nerve is not involved, and you retrieve the piece:
- We may be able to reattach it if it’s a clean break.
- Otherwise, we can restore it with a filling or crown.
- Root Fracture
- Vertical root fractures usually require extraction.
- Horizontal fractures might be saved with root canal treatment (RCT).
- Tooth Avulsion (Completely Knocked Out)
- Do not touch the root. Handle the tooth by the crown (the part you see in the mouth).
- Place it in a cup of milk and call our office immediately.
- We may be able to reinsert and splint the tooth, followed by a root canal at a later time.
- Bone Fracture
- If the surrounding bone is fractured, we may need to stabilize the area or refer to an Oral & Maxillofacial Surgeon.
- Tooth Mobility
- If the tooth is loose, we may need to stabilize it with a flexible wire while the area heals.
- Nerve Involvement
- If the nerve is exposed:
- In a fully developed tooth, we may recommend a root canal.
- In an immature tooth, we might perform an apexogenesis to promote continued root growth.

Dental trauma is all about timing. The sooner we treat the injury, the better the outcome. Trauma cases often require multiple follow-up visits to monitor healing and provide ongoing care.
